ABOUT CPLC Corazón
CPLC Corazón is a client-centered, multicultural center with bilingual staff offering a variety of treatment modalities. We have been providing Substance Abuse treatment through our CARF Accredited Level II Residential Treatment Center since 1983. The 30- to 60-day structured treatment program incorporates best practices targeted to the clients enrolled in the program.
HOW WILL YOU MAKE AN IMPACT?
As the Integrated Care Manager, you will carry a caseload of current clients in the Corazón Substance Abuse Residential Treatment Program to support the diminishment of Social Determinants of Health (SDOH). You will get to collaborate with all program staff on the Corazón campus. Your work will include communicating and coordinating with both internal and external partners, connecting clients with resources and services to address SDOH needs as identified during intake through the use of the PRAPARE, and creating a plan prior to discharge to assist clients with connecting to community resources and other service providers.
- Conduct comprehensive intake assessments using PRAPARE, effectively identifying client SDOH needs and establishing individualized care plans within 48 hours of intake.
- Manage a diverse caseload, maintaining timely documentation, tracking progress, and ensuring clients stay connected to the resources and supports that reduce SDOH barriers.
- Collaborate closely with clinical staff, program teammates, and external partners, keeping everyone informed while coordinating referrals, discharge plans, and community linkages.
- Respond effectively to crisis situations, providing on‑call, weekend, or holiday support as scheduled to ensure client safety and continuity of care.
- Travel into the community if needed to meet partners, providers or clients.

MINIMUM QUALIFICATIONS AND COMPETENCIES
- Must meet one of the criteria for a Behavioral Health Technician below:
- Master's degree or bachelor's degree in a field related to behavioral health, or
- Registered nurse, or
- Physician assistant who is not working as a medical practitioner, or
- Bachelor's degree and at least one year of full-time behavioral health work experience, or
- Associate's degree and at least two years of full-time behavioral health work experience or
- High school diploma or high school equivalency diploma and credit hours of post-high school education in a field related to behavioral health completed no more than four years before the date the individual begins providing behavioral health services and two years of full-time behavioral health work experience or four years of full time behavioral health work experience, or
- Licensed as a practical nurse, according to A.R.S. Title 32, Chapter 15, with at least two years of full-time behavioral health work experience.
- Experience working in a residential setting
- Knowledge of behavioral health systems and community resources
- Ability to work with unique populations such as seriously mentally ill, homeless, and/or justice connected men.
- Must have or obtain within 30 days of hire: CPR/First Aid Certification, CPI certification, AZ Fingerprint Clearance Card, current TB test.
- Possess an Arizona Driver’s license, vehicle insurance and ability to be covered under CPLC policy
- Bilingual a plus.
