Through the Children’s Long-Term Support (CLTS) Waiver Program, you will help children who have substantial limitations in their daily activities remain in their homes. You will develop partnerships with families to jointly design a plan that addresses the participant's needs and meets his/her desired individual outcomes. This is a unique chance to work with participants to help them achieve optimum physical and mental health and optimal social functioning to reach maximum independence and community participation.

Qualified candidates will be licensed or certified social worker and have a minimum of one year of experience working with children and/or youth with disabilities. Other persons appropriately qualified would have an associate's degree in a human services-related field and two years of experience working with persons of a specific target group for which they are employed.

You must possess excellent assessment skills and organizational skills. Equally important is your ability to work independently in challenging situations. You will need an aptitude for communication, both verbal and written.

The successful candidate will be required to become certified in the Children’s Long-Term Care Functional Screen within 30 days of employment. New hires are required to hold a valid driver’s license, pass a pre-employment drug screen and background check.

Sawyer County Position Description

Title: Children’s Long-Term Support Service Coordinator

Department: Sawyer County Health and Human Services, Behavioral Health Department

Reports to: Child Welfare & Supportive Services Supervisor

Position Purpose:

The Children’s Long-Term Support (CLTS) Waiver Program is a home and community-based service that provides Medicaid funding for children who have substantial limitations in their daily activities and need support to remain in their home or community. The program follows waiver requirements and guidelines per State Statute 46.27(6r) (a). CLTS falls within the Children, Youth & Families Division and is part of the Children’s Supportive Services Department. The purpose of the CLTS Service Coordinator is to provide assistance and support to children and youth with disabilities and their families to achieve an inclusive, interdependent, and self-empowered life within their own home and community in accordance with the Children’s Long-Term Support (CLTS) program. As with all positions within our agency, it is the duty of the individual in this position to fully support the mission, vision, and values of the agency, to work in a coordinated manner with other employees, and to demonstrate professional and personal confidence in the service delivery model and approaches taken by the organization. The CLTS SSC is expected to integrate trauma-informed care while serving our communities.

Duties and Responsibilities:

The SSC will develop a working partnership between the participant (and parent(s)/guardian(s)) to jointly develop a plan that addresses the participant’s identified needs and meets his/her desired individual outcomes. While keeping the participant’s preference in mind, the CLTS SSC will seek the most cost-effective means to meet those individual outcomes in an integrated community setting. The Medicaid waiver programs are built upon a foundation of primary program values. These values support individual choice, the enhancement of relationships, the building of accessible, flexible service systems, the achievement of optimum physical and mental health for the participant, and the promotion of presence, participation, and optimal social functioning in the community. The program values further seek to ensure that participants are treated with respect and that service systems empower individuals, build on their strengths, enhance their self-worth, and provide the tools necessary to achieve maximum independence and community participation. The duties and responsibilities described are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

· Complete/update functional screens, monthly collateral contacts, six-month reviews, recertification, and required face-to-face contacts in a manner that is trauma-informed, culturally relevant, and non-stigmatizing.

· Write and implement care plans for Children’s Long Term Care Waiver.

· Assist participants and their families in obtaining benefits through the Northern Income Maintenance Consortium.

· Utilize effective strategies to engage program participants, enhance motivation, and encourage meaningful collaborative and strength-based goal planning and progress.

· Maintain contact with program participants to a degree sufficient to support effective engagement, as well as monitor and coordinate the activities of the individual service plan providers. Assess for additional needs and assist in obtaining the most successful outcomes for the participant as possible.

· Fiscally responsible for weekly decisions made on expenditures of allowable services for each participant.

· Assist the program participant with identifying and establishing natural and professional supports in the community.

· Function as an advocate on behalf of the CLTS participants and their families.

· Arrange staffings in a timely fashion to ensure the appropriateness of services rendered and to make modifications to the care plan as needed.

· Plan, inform, and coordinate discharge planning with participants and their families.

· Provides transitional services for children turning eighteen years (18) old; i.e., guardianship, adult disability application for Social Security, and referrals to Family Care System.

· Maintain confidential, organized, detailed, and carefully documented case files for each client/case enrolled in either the CLTS Program.

· Provides Targeted Case Management (TCM) on a monthly basis to the Fiscal Department.

· Document critical incidents and restrictive measures.

· Completion of Third Party Administrator authorizations.

· Maintain professional development by attending trainings or conferences to enhance case practice skills, methods of providing services to the consumers and their families, and be current in the latest developments in the CLTS Program.

· Provide information regarding the CLTS Program to the public, other service agencies, and interested individuals.

· Perform all other duties as assigned by the Behavioral Health Clinic Supervisor/ CCS Services Director.

Qualifications:

Education and Experience:

The SSC must be a licensed or certified social worker and have a minimum of one year of experience working with children and/or youth with disabilities. Other qualifications for this position include:

· The skills and knowledge typically acquired through a course of study and practice experience that meets requirements for state certification or licensure as a social worker, and also one year of experience with the target group(s) for which they are employed.

· A course of study leading to a Bachelor of Arts or Bachelor of Science degree in a health or human services-related field and one year of experience working with persons of the specific target group(s) for which they are employed.

· An associate’s degree in a human services-related field and two years of experience working with persons of the specific target group(s) for which they are employed.

· Any combination of training in a health or human services-related field and experience in long-term care case management and/or case management for individuals with disabilities or special needs that totals four years. Examples include but are not limited to: One year of coursework or training in a human services-related field and three years of experience in long-term care case management, or four years of experience in long-term support case management.

• Proficient computer skills, specifically in current versions of Microsoft Word, Excel, Outlook, and PowerPoint; and willingness to learn new skills as needed or required

• Accept agency philosophy, purpose, and objectives

• Must pass a caregiver background check and urine screen

• Valid motor vehicle operator’s license and proof of insurance

· Certified in the Children’s Long-Term Care Functional Screen within 30 days of hire

Ability to operate a variety of office equipment, including computer terminal, typewriter, telephone, fax machine, calculator/adding machine, and photocopier. Ability to coordinate eyes, hands, feet, and limbs in performing movements requiring moderate skill, such as typing. Ability to sustain prolonged visual concentration. Ability to exert light physical effort in sedentary to light work, which may involve some lifting, carrying, pushing, and pulling.

Environmental Adaptability

Ability to work under safe conditions where exposure to environmental factors, such as repetitive computer keyboard use, poses a very limited risk of injury.
