• Per-Diem Social Worker (LLMSW, LMSW)
• Administrative Support

In these roles, you’ll:

• Provide care and support to patients and families during a variety of different schedules
• Triage and respond to after-hours patient needs via phone and in-person visits
• Collaborate with an interdisciplinary team to develop and implement plan of care
• Conduct informational visits and complete possible assessments as needed
• Deliver exceptional, holistic care that honors the end-of-life journey

Standard Per-Diem Requirements:

• 2 shifts a month
• 2 weekend shifts per quarter (every 3 months)
• 1 holiday a year

Requirements:

• License/Certificated in the state of MI
• Home Health, Assisted living or Skilled nursing facility, Long-term care, Nursing Home or Rehab
• Acute care, Hospital, ICU, Urgent Care
