The CPLC Supportive Housing Services team is Empowering Lives through Housing Stability. We empower lives by actively engaging individuals and families experiencing homelessness or housing instability, particularly those facing multiple barriers to care and housing access.

HOW WILL YOU MAKE AN IMPACT IN THIS ROLE?

  • You will focus on conducting proactive, community-based and street outreach to locate, engage, and connect clients to essential services, including affordable housing opportunities and supportive resources.
  • You will be responsible for orienting eligible clients to available programs, facilitating connections to housing navigation and service coordination supports, and ensuring successful linkage to ongoing care.

SPECIALIZATIONS

Outreach & Community Engagement
• Build and maintain strong relationships with community partners, landlords, and service providers.
• Conduct proactive, street-based outreach using best practice standards to engage individuals experiencing homelessness.
• Locate and re-engage referred members through field-based and community outreach efforts.
• Represent the organization at community events, encampments, shelters, and partner locations.
• Develop and maintain a comprehensive resource directory of community services.

Documentation & Compliance

• Complete timely and accurate documentation of all client interactions and services provided.
• Enter all client engagements into HMIS and Unite Us within 24 hours of service delivery.
• Ensure documentation meets all program, agency, and compliance standards.
• Maintain clear, objective, and detailed case notes that reflect services provided, client progress, and next steps.
• Develop and document all client goals and engagement notes using the SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goal format.
• Ensure all records are up to date, audit-ready, and in alignment with data quality expectations.

Service Facilitation
• Establish links to community agencies and service providers.
• Provide basic case management and referral services.
• Monitor the ongoing provision of services from outside agencies.

Research and Evaluation
• Analyze the types, frequency, and characteristics of services clients use and need.
• Study available community services and eligibility requirements.
• Identify service gaps, barriers to access, and opportunities for improved outreach strategies.
• Observe client needs and trends to inform program development.

MINIMUM QUALIFICATIONS AND COMPETENCIES

Education /Background:

  • Bachelor's degree preferred
  • Two years of experience in housing, case management or social service delivery.
  • A combination of a minimum of six years of work experience and education will be considered.
  • This position requires driving on behalf of the company. Candidates must hold a valid driver’s license, meet the company’s motor vehicle record (MVR) standards, meet company’s insurance eligibility, and comply with all state and federal driving requirements. Ongoing employment is contingent upon maintaining an acceptable driving record.
  • This position includes duties subject to age‑based legal requirements. Therefore, candidates must be at least 21 years of age to perform essential job functions and to comply with federal and state regulations governing the work performed in this role.
  • Employment in this position is contingent upon successful completion of all required pre‑employment screenings and subject to credential checks to maintain compliance with licensing and contractual requirements.
      • Fingerprint-based background check in accordance with federal, state, and contract requirements.
      • Required immunizations or proof of vaccination
        • TB test prior to start

  • Experience and commitment to working with multiethnic, multicultural, low-income populations.
  • Excellent attention to detail, analytical, and problem-solving skills.
  • Demonstrated interpersonal and verbal and written communication skills.
  • Proficient with Microsoft Office (Word, Excel, PowerPoint, Outlook, especially Microsoft teams).
  • Excellent documentation and clinical skills.
  • Proven ability to follow up and meet deadlines.
  • Excellent initiative, flexibility, and stress-management in handling the demands of leadership.
