Role Overview:
Reporting to the Clinical Supervisor, the Case Manager provides culturally responsive, client-centered care as part of a multidisciplinary team for individuals with complex mental health and psychosocial needs.
This role includes short-term post-crisis follow-up to support stabilization, reduce crisis recurrence, and connect clients to ongoing services.
The Case Manager will assist with referrals, care coordination, and system navigation across multiple systems, including hospitals, crisis services, housing programs, legal systems, schools, and medical providers
Key Responsibilities:
• Manage assigned caseload while delivering high-quality, client-centered care
Provide 5+ hours of direct services daily, including outreach and engagement
Conduct rapid follow-up and ongoing support for individuals post-crisis discharge
Deliver field-based services in homes, shelters, hospitals, and community settings
Assess risk, needs, and barriers; develop short-term stabilization plans
Coordinate care and link clients to mental health, medical, and substance use services
Facilitate appointments and warm handoffs to ensure continuity of care
Collaborate with multidisciplinary teams and external partners
Support clients in accessing housing, food, transportation, ID, and public benefits
Advocate to reduce systemic barriers and improve service access
Coordinate across systems (healthcare, legal, housing, social services)
Engage families and natural supports to strengthen care networks
Provide culturally responsive, trauma-informed services
Maintain accurate documentation and full regulatory compliance
Support successful transitions to ongoing care and reduce crisis re-entry
Qualifications:
• Bachelor’s degree in social, behavioral, or human services field required.
• Must obtain Washington State Agency Affiliated Counselor credential.
• Experience working with high-acuity populations, including individuals experiencing crisis, homelessness, or complex behavioral health needs preferred.
• Knowledge of behavioral health systems, crisis response systems, and community resources in King County.
• Strong care coordination, advocacy, and system navigation skills.
• Ability to work in community-based settings with a high level of independence and accountability.
• Understanding of trauma-informed care and recovery-oriented practices.
• Demonstrated ability to provide culturally responsive services.
• Bilingual (English/Spanish) preferred.
• Valid driver’s license, reliable transportation, and acceptable driving record.
• Proficient in Microsoft Office and electronic health records.
WORK SCHEDULE
This Full-time in-person position works a typical schedule of 8 AM - 5 PM, Monday to Friday.
SALARY RANGE
$ 27.00 to $ 32.45 an hour, depending on experience, qualifications, and credentials.
