Citizens Memorial Hospital (CMH) in Bolivar, MO is hiring a Licensed Practical Nurse (LPN, LVN) to join their dynamic team providing services to our Home Health patients. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!

A typical day includes : the opportunity to focus on individualized patient treatment in their home.

• Developing a relationship with our patients and supportive healthcare team
• Seeing 5-6 patients daily to provide skills in the home.

About the department: The goal of the Home Health Department is to decrease the number of hospital days and hospital visits per patient by providing skilled nursing and other Home Health services in the patient’s home. Also, to instruct and assist patients and their families regarding their diagnosis, new procedures, medications, prevention of complications, control of pain, and general home care. Individualized plans of care are developed and used to assess, intervene, and treat home-bound patients who may not otherwise receive the healthcare they need due to physical, mental, or socio-economic circumstances.

Job Requirements:

• Education: Associates
• Experience: prior home health experience preferred but not required
• Skills: Multitasking, compassion, organization
• Licensure/Certification: LPN
• Physical: the ability to stand for long periods and to thrive in a fast-paced environment

The position is: Part Time, Days Weds, Thurs, Fri
