Provides direct patient care and/or case management within a patient and family centered care environment. Provides services requiring substantial and specialized nursing skill, initiates appropriate preventive and rehabilitative nursing procedures, prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the patient's condition and needs.

RESPONSIBILITIES:

• Develops and revises plan of treatments and discharge plan in conjunction with patient, family, physician, care delivery team and third party payors including obtaining medical orders and approval of medical services.
• Consults with and educates the patient, family and other team members regarding disease process, self care techniques and preventive strategies.
• Provides supervision of LPN to ensure patient health, safety, and compliance with plan of care.
• Demonstrates flexibility and willingness to visit patients based on need and not geography.
• Maintains productivity requirements.
• Ensures that appropriate visit authorizations are in place and that visit documentation reflects the medical necessity for care delivery and justification.
• Performs other duties as requested.

QUALIFICATIONS:

1. New York State Licensed RN.
2. One year of experience as a clinical pediatric nurse required, home care experience preferred.
3. Strong interviewing, assessment, and communication skills.
4. Current NYS driver’s license.

Physical Requirements: The position involves frequent sitting, standing, and walking. Occasional bending and pushing may be required. Tasks such as kneeling, squatting, reaching, twisting, balancing, climbing, and pulling are required infrequently.

Salary - $73,652 - $103,854

The referenced pay range represents the minimum and maximum compensation for this job family. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
