About the Role:

The MDS/RN or LPN plays a critical role in ensuring the delivery of high-quality patient care through comprehensive assessment and care planning. This position is responsible for conducting thorough Minimum Data Set (MDS) assessments to evaluate residents' health status, functional capabilities, and care needs. The MDS/RN collaborates closely with interdisciplinary teams to develop individualized care plans that promote optimal health outcomes and regulatory compliance. This role requires meticulous documentation and adherence to federal and state guidelines to support reimbursement processes and quality improvement initiatives. Ultimately, the MDS/RN serves as a key liaison between clinical staff, residents, and families to foster a supportive and effective care environment.

Position: MDS Specialist (RN) (LPN)

Location: Mt Vernon, Mo


Responsibilities:

  • Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA and other payor program requirements
  • Ensure accuracy of RAI to reflect optimal reimbursement for services provided
  • Ensure appropriate documentation to report and support services provided and assessment accuracy


Requirements:

  • Current RN or LPN licensure

  • Previous MDS 3.0 experience required.

The MDS/RN utilizes clinical assessment skills daily to gather accurate and comprehensive resident data, which forms the foundation for care planning. Analytical skills are essential to interpret assessment results and identify changes in resident conditions that require intervention. Strong communication skills facilitate collaboration with interdisciplinary team members and clear explanation of care plans to residents and families.
