Summary:
Responsible for coordinating care for patients receiving medical and mental health services in accordance with established policies and procedures. The discharge planner facilitates applications for MassHealth benefits and ensures seamless coordination of
care with outside providers in preparation for a patient’s release from the facility. This is an excellent entry-level opportunity for someone looking to begin a career in healthcare support services.
Responsibilities:
• Discharge Planner is responsible for discharge planning for patients receiving mental health services, particularly patients diagnosed with a Severe Mental Illness (SMI) or patients with a serious medical illness, in accordance with the policy and procedures.
• The discharge planner also coordinates aftercare for patients releasing to the community for a variety of services, i.e. scheduling PCP visits, exploring housing options and employment.
• The discharge planner must facilitate applications of eligibility for SSDI, SSI, or other available forms of financial assistance, as well as resumption of benefits for patients awaiting release from the facility.
• Works within a multi-disciplinary team to ensure a smooth transition post release
• Sustains positive working relationships with community providers
• Coordinates other areas of aftercare, as needed
Schedule:
Full Time
Monday–Friday, 9:00 AM–5:30 PM
Qualifications:
• High School Graduate
• Must be able to pass a facility background check
• Frequent use of a computer, including typing data entry, and reviewing electronic medical records
• Ability to communicate clearly in person, over the phone, and via electronic communication systems

- Occasional standing and walking throughout the facility to meet with patients and staff
- Ability to navigate a correctional facility environment, including passing through secured areas and multiple units
- Ability to lift and carry up to 10–15 pounds (e.g., files, office materials)
- Sufficient visual acuity to read written documentation and computer screens
- Ability to hear and understand verbal communication in a variety of settings
