Salary Range: $154,467.04 To $190,200 Annually

Hours are Monday through Friday, 8:00 AM - 4:30 PM. Position is based at Harbor Community Health Center location in Plymouth. Position requires up to 75% travel to community health centers in MA.

Responsibilities:

Provide drug therapy and other pharmaceutical care services in collaboration with multi-disciplinary teamCoordinate oral and written consultation with other health care professionals regarding drug therapy selection and managementDevelop patient-specific drug therapy management plans and therapeutic endpointsPerform medication therapy management pharmacotherapy consultations and assessmentsCoordinate care of patients between community health centers and in-house and retail pharmaciesRequires up to 75% travel to community health centers in MA

Requirements:

Doctor of Pharmacy (PharmD) from ACPE-accredited institutionLicense to practice pharmacy in MABLS certificationPharmacy Residency
