About This Position

JOB SUMMARY |
- The social worker is responsible for assisting in meeting the patient’s needs throughout the continuum of care. The Social Worker guides patients, families, physicians and the interdisciplinary team to the appropriate community and adjunct resources that foster quality of life. The social worker interviews patients/families/caregivers/support systems to obtain an age-appropriate psychosocial assessment. The social workers assists patients/families/caregivers/support systems in adjustments to illness, disabilities and the resolution of difficulties within the care management process. The social worker provides psychosocial assessments and develops an interdisciplinary plan of care with the patient and stakeholders to best meet the patient/family needs. The social worker implements and assists with discharge planning services particularly for the complex patient.

JOB REQUIREMENTS |
- Minimum Education
- Bachelor’s Degree in Social Work with current LBSW.
- Master’s Degree in Social Work preferred with LMSW or LCSW.
- Minimum Work Experience
- One (1) year in healthcare setting.
- One (1) year in hospital performing medical social work preferred.
- Required Licenses/Certifications
- Licensed Social Worker in State of Texas within 6 months of hire.
- Certification through Commission for Case Manager (CCM) or American Case Management Association (ACMA) within 2 years of hire.
- Required Skills, Knowledge, and Abilities
- Ability to demonstrate advanced communication and interpersonal skills with all levels of internal and external customers, including but not limited to medical staff, patients and families, clinical personnel, support and technical staff, external agencies, and members of the community.
- Ability to show strong commitment to interdisciplinary collaboration and communication.
- Ability to demonstrate strong computer skills.
- Ability to demonstrate excellent human relations skills.
- Ability to show creative problem solving.
- Ability to be proficient with organization and planning.
- Ability to show initiative and work with no supervision.
- Ability to tolerate frequent interruptions and demanding workload.
- Proficient in Allscripts EMR, Allscripts CM software program, IDX and 3M within 6 months.
