Description:
LINE OF RESPONSIBILITY: Behavioral Health Clinician reports to the Director of Behavioral Health, hereafter referred to as Director.
PURPOSE: To provide comprehensive quality behavioral health care for HCHC patients.
RESPONSIBILITIES:
- Develop behavioral health expertise which will integrate and support patient care in the primary care setting. Expertise includes a clinical understanding and approach to the relationship between behavioral health, medical diagnosis, and condition.
- Provide, complete, and document all patient psychosocial assessments, diagnosis, treatment planning and psychotherapy/counseling to individuals and families using electronic medical records. Service may be provided by telehealth or in person visits.
- In collaboration with the Behavioral Health Care Coordinators, submit insurance re-authorizations and complete EHR encounter notes in a timely manner to ensure continued patient care and reimbursement.
- Assume responsibility for inter- and intra-agency collaboration on cases, including verbal and written reports to other agencies, court appearances, and communication with other professionals regarding patient care.
- Provide crisis intervention, case management and referral services as appropriate.
- Participate in weekly clinical meetings, present cases, provide consultation unless exempted by Director.
- Work integratively with all medical providers to improve patient care using behavioral health interventions.
- In collaboration with Behavioral Health Coordinators and the Behavioral Health Director, maintain a full schedule in order to meet productivity standards of clinically reimbursed hours specified in BH Clinician contract.
- Respond promptly to all patient and agency related communications.
- Other related duties as assigned by Director.
QUALIFICATIONS:
- Master’s level degree, with current license to practice independently in Massachusetts or to provide service under the direct supervision of an LICSW.
- Ability to establish effective, positive working relationships, internal and external to the Organization.
- To conduct oneself within established professional and ethical boundaries
