Reports to: Operational Manager
Hours: Full-time, 37.5 hours per week (rota basis for emergency phone)
Contract: Permanent
Probation: 6-month probation period
Salary: 30,000 33,000 per annum, subject to qualification and experience

Job Purpose
The Care Coordinator is the human heart of each persons support journey. Individualised care plans for the children and adults we support, working closely with families, support workers, and external professionals.
This role uses our integrated care platform to handle scheduling, documentation, and routine administration freeing you to focus on what matters most: relationships, judgement, advocacy, and getting the details right for each individual.
Key Responsibilities
Personalised Care Planning
Develop, review, and adapt person-centred care plans for children and adults with complex needs.
Conduct regular reviews with the individual, their family, and their wider support network.
Translate clinical, educational, and therapeutic recommendations into practical day-to-day support.
Coordination and Oversight
Match support workers to individuals based on skills, personality, and continuity of care.
Oversee rotas, handovers, and transitions, ensuring consistency for each person.
Act as the main point of contact for families, escalating concerns promptly and sensitively.
Manage client care and support requests from start to finish, ensuring workers are allocated based on skills, availability, and client preferences.
Act quickly to resolve last-minute staffing changes or placement issues.
Quality, Safeguarding, and Compliance
Ensure all care delivery meets CQC standards and internal quality benchmarks.
Maintain accurate records and contribute to audits, inspections, and continuous improvement.
Uphold safeguarding responsibilities for both children and adults at risk.
Promote dignity, choice, and the rights of the individuals we support.
Collaboration & Service Improvement
Liaise with social workers, health professionals, schools, day services, and commissioners.
Work closely with the wider team to share learning and develop best practice.
Collaborate with colleagues and management on new initiatives, service improvements, and business development projects.
Work towards agreed monthly and quarterly goals, with performance incentives in place.
Person Specification
Essential
Demonstrable experience coordinating care or support for people with disabilities, additional needs, or complex health conditions (children, adults, or both).
Strong understanding of person-centred practice and the principles of choice, dignity, and inclusion.
Excellent communication and relationship-building skills, with the confidence to advocate for individuals and have difficult conversations with families and professionals.
Strong organisational and multitasking abilities; able to remain calm and solution-focused under pressure.
Sound judgement and the ability to make decisions under pressure.
Comfortable using digital tools.
Knowledge of safeguarding requirements for children and adults at risk.
Right to work in the UK and willingness to undergo an enhanced DBS check.
Desirable
Relevant qualification in health, social care, nursing, social work, or a related field (e.g., NVQ Level 3+, Diploma in Health and Social Care).
Previous experience in placement coordination, recruitment, or healthcare staffing.
Experience working within CQC-regulated services.
Familiarity with care planning software, electronic records, or AI-assisted tools.
Driving licence and access to a vehicle.
