Description:

Park DuValle Community Health Centers is seeking an Advanced Practice Registered Nurse (APRN) to provide comprehensive primary care services to adult patients. The APRN will evaluate, diagnose, treat, and manage patients' health conditions while providing preventive care, health education, and ongoing care coordination.

The position requires sound clinical judgment, strong communication skills, and the ability to work collaboratively with physicians and other members of the healthcare team. The APRN will provide care in accordance with Kentucky laws and regulations, established clinical standards, and Park DuValle policies and procedures.

Responsibilities

  • Provide comprehensive primary care services to adult patients within the APRN's scope of practice and in accordance with Kentucky statutes, regulations, and Park DuValle policies.
  • Obtain and document complete medical histories and perform comprehensive physical examinations, including assessment of biological, social, and psychological health needs.
  • Diagnose, treat, and manage acute and chronic medical conditions and develop individualized treatment plans based on patient needs.
  • Provide preventive healthcare services, health screenings, patient education, and counseling to promote overall health and wellness.
  • Prescribe medications and treatments within the APRN's scope of practice and established clinical privileges.
  • Order, perform, and interpret appropriate laboratory and diagnostic tests and incorporate results into the patient's treatment plan.
  • Monitor patient progress and modify treatment plans as clinically appropriate.
  • Refer patients to physicians, specialists, community resources, and other healthcare services when indicated.
  • Provide case management and care coordination for patients as appropriate.
  • Maintain accurate, timely, and complete patient documentation in the electronic health record (EHR) and other required records.
  • Participate in quality improvement activities, clinical initiatives, and programs designed to improve patient outcomes and quality of care.
  • Collaborate with physicians, nurses, behavioral health providers, care management staff, and other members of the interdisciplinary healthcare team.
  • Participate in medical staff meetings, team conferences, continuing education, and other required meetings and training.
  • Maintain patient confidentiality and comply with HIPAA, applicable federal and state regulations, and Park DuValle policies and procedures.
  • Perform other clinically and administratively related duties as assigned.
Requirements:

  • Clinical: Adult primary care, patient assessment, diagnosis, treatment, medication management, and preventive care.
  • Interpersonal: Communication, patient relations, teamwork, collaboration, and confidentiality.
  • Intellectual: Clinical judgment, critical thinking, problem-solving, decision-making, and interpretation of diagnostic information.
  • Professional: Accountability, adaptability, organization, professionalism, and commitment to quality patient care.

Education:

  • Master’s degree in nursing (MSN) from an accredited nursing program.
  • Graduate-level education appropriate to the APRN’s area of practice.

Professional:

  • Current, unrestricted Kentucky APRN license.
  • Current national board certification appropriate to the area of practice.
  • Prescriptive authority in Kentucky.
  • Current BLS certification.
  • Completion of required continuing education.
  • Completion of required in-service training.
  • Must meet Park DuValle credentialing and clinical privileging requirements.

Physical/Environmental

  • Ability to perform the essential functions of the position with or without reasonable accommodation.

Additional Requirements

  • Adult primary care experience preferred.
  • Experience caring for patients age 19 and older.
