Affinity Healthcare Solutions Skilled Care is seeking compassionate, motivated, and patient-focused Occupational Therapists to join our growing Home Health team throughout Philadelphia, Montgomery, Delaware, Chester, Bucks, and Lehigh Counties.

Responsibilities

• Perform comprehensive occupational therapy evaluations in the home setting
• Develop individualized treatment plans based on patient goals and clinical needs
• Provide skilled occupational therapy services in patients' homes
• Improve patients' ability to safely perform Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
• Recommend adaptive equipment and home modifications to enhance safety and independence
• Educate patients and caregivers on therapeutic techniques, fall prevention, energy conservation, and home safety
• Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals
• Monitor patient progress and modify treatment plans as appropriate
• Maintain accurate and timely clinical documentation
• Complete all patient notes and communications within 24 hours
• Ensure compliance with agency policies and applicable regulations
• Travel to patient homes throughout assigned coverage areas

Qualifications

• Active Pennsylvania Occupational Therapist (OT) License
• Graduate of an accredited Occupational Therapy program
• Home Health experience preferred
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal skills
• Strong computer and documentation skills
• Reliable transportation and a valid driver's license
• Ability to work independently and effectively manage a flexible schedule
• Commitment to delivering exceptional patient-centered care
