Job Description
Position Purpose:
This position is intended to follow the patient beyond the post-acute visit, coordinating care across the continuum.
The Care Manager works with individuals/families/support systems to improve the overall health and well-being of the patient. The Care Manager will facilitate care through a collaborative process to meet an individual/families comprehensive health needs through various assessments, care coordination, facilitation, and advocacy for options and services to promote quality care, patient safety, and cost-effective outcomes. The Care Manager will follow standards of practice for case management including those published by CMSA and/or ACMA. The Care Manager will use clinical expertise and problem-solving skills to achieve optimal clinical and resource outcomes within effective time frames.
Specifications (Education, Knowledge, Skills, Abilities, Licensure, Certification, Registration):
- Graduate of an accredited School of Nursing required.
- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Missouri required. Candidate must possess an Illinois nursing license or be willing to obtain one within six (6) months of hire
- Care Management experience preferred
- BLS (CPR) required or obtained within 90 days of hire
- Certification in Case Management preferred
- Experience working in OB/GYN, Pediatrics, or Care Management of maternal-child populations preferred
- Strong verbal and written communication and facilitation skills required
- Strong assessment skills, time management skills, critical thinking skills required
- Advanced computer skills required
- Must have ability to interact effectively with a variety of people and situations at all levels of the organization
- Possess a valid driver’s license and reliable transportation
- Candidates should possess a positive attitude, have a high level of energy, work in a team environment, and be resourceful for an evolving program
