About the Role

As a Mental Health Counselor/Assessor, you will provide behavioral health assessments, diagnosis, treatment planning, counseling services, crisis intervention, and care coordination for individuals living with Severe and Persistent Mental Illness.

This position combines clinical assessment with ongoing therapeutic support, allowing you to build meaningful relationships while helping clients achieve their recovery goals.

Services may be provided in the office, in clients' homes, or within community settings when clinically appropriate.


What You'll DoClinical Assessment & Treatment Planning
  • Complete initial and annual mental health assessments
  • Establish DSM diagnoses
  • Develop collaborative, recovery-oriented treatment plans
  • Maintain documentation that meets Medicaid and agency standards
Counseling & Therapeutic Services
  • Provide individual counseling services
  • Facilitate group counseling and psychoeducation
  • Utilize evidence-based therapeutic interventions
  • Support individuals through recovery and wellness planning
Crisis Response & Care Coordination
  • Conduct face-to-face crisis assessments and interventions
  • Assist individuals through transitions of care
  • Coordinate services with healthcare providers, community partners, and support systems
  • Advocate for client needs across systems of care
Documentation & Compliance
  • Complete timely and accurate clinical documentation
  • Prepare authorization requests and utilization reviews
  • Utilize Electronic Health Record (EHR) systems effectively
  • Maintain ethical, legal, and professional standards
Requirements:
  • Master's degree (MA/MS) in Counseling, Social Work, Psychology, or related behavioral health field
  • QMHP certification through MHACBO, or ability to obtain certification and demonstrate active pursuit of licensure
  • Ability to assess, diagnose, develop treatment plans, and provide therapeutic interventions
  • Valid Oregon driver's license
  • Ability to meet background check and agency hiring requirements
Preferred Qualifications
  • Experience working with individuals experiencing Severe and Persistent Mental Illness (SPMI)
  • Experience supporting individuals with co-occurring mental health and substance use disorders
  • Experience conducting assessments, DSM diagnosis, and treatment planning
  • Familiarity with Medicaid documentation standards
  • Experience providing community-based or in-home behavioral health services
  • Strong interdisciplinary collaboration and care coordination skills
  • Bilingual or multilingual abilities
