Join Bethany Communities as a Resident Service Coordinator (RSC) and become an essential member of our Resident Care team. In this rewarding role, you'll serve as the primary connection between residents and the supportive services they need to live safely, confidently, and with dignity. From coordinating healthcare and wellness services to building meaningful community partnerships, you'll have the opportunity to make a lasting impact in the lives of those we serve. We are excited to welcome a Part-Time (20 hours per week) Resident Service Coordinator to support residents at one of Haverhill communities.

What You'll Do

  • Embrace and promote Bethany's mission, vision, and values in every interaction.
  • Serve as the primary resource for residents seeking supportive services, including healthcare, wellness, transportation, meals, personal care, adult day programs, and more.
  • Provide general case management, intake, assessments, and referrals to appropriate community resources.
  • Conduct comprehensive resident assessments, identify needs, and develop individualized service plans when appropriate.
  • Educate residents and families about available services, eligibility requirements, application processes, and resident rights.
  • Advocate for residents while coordinating services with internal teams and external agencies.
  • Monitor ongoing services and maintain communication with providers to ensure residents receive quality care.
  • Collaborate closely with Bethany's Wellness Nurses and Certified Nursing Assistants to coordinate resident care.
  • Partner with physicians, family members, insurance providers, Area Agencies on Aging (AAA), Aging Service Access Points (ASAP), and other healthcare professionals.
  • Build and maintain strong relationships with hospitals, rehabilitation centers, skilled nursing facilities, home health agencies, home care providers, hospice organizations, and other community partners.
  • Coordinate care plans and communicate effectively with the Wellness Team to ensure continuity of care.
  • Represent Bethany at community outreach and marketing events, promoting our housing and supportive service programs.
  • Encourage resident engagement by helping build informal support networks among residents, families, and friends.
  • Provide guidance and education to management staff on aging in place, service coordination, and resident support when appropriate.
Requirements:

What You Bring

  • Knowledge of federal and state programs, including Medicare, Medicaid, MassHealth, Group Adult Foster Care, PACE, Senior Care Options (SCO), Home and Community-Based Waiver Programs, and the Massachusetts State Home Care Program.
  • Excellent organizational, written, verbal, and interpersonal communication skills.
  • The ability to build strong relationships with residents, families, healthcare providers, and community organizations.
  • Strong problem-solving abilities and the ability to manage multiple priorities effectively.
  • A collaborative approach and the ability to work successfully within a multidisciplinary team.
  • A commitment to ongoing professional development and lifelong learning.

Qualifications

  • Bachelor's degree in Social Work, Gerontology, Psychology, Counseling, or a related field.
  • Applicants without one of the listed degrees will be considered if they possess relevant experience in resident service coordination or a related field.
  • Two to three years of experience providing social services to older adults is preferred.
  • Working knowledge of community resources and supportive services available to seniors.
  • Demonstrated ability to advocate for residents, coordinate services, organize resources, solve problems, and achieve positive outcomes.
  • Training in aging services, disability services, federal and state entitlement programs, legal liability related to service coordination, mental health, and substance use is highly desirable.
  • SHINE certification a plus.
