Essential Duties and Responsibilities
Personal Care Services
Per 12VAC5-381-360(B), the CNA / Home Attendant provides assistance with the following as specified in the client’s Individualized Service Plan (ISP):
- — Bathing, showering, and personal hygiene — including hair care, oral care, nail care, and skin care
- — Dressing and grooming assistance
- — Toileting, incontinence care, and use of assistive devices
- — Ambulation, mobility assistance, and safe transfer techniques
- — Feeding and meal assistance as directed
- — Medication reminders (non-skilled verbal cues only — CNA does not administer medications)
- — Taking and recording vital signs when specified in the ISP
- — Companionship, conversation, and emotional support
Instrumental Activities of Daily Living (IADLs)
- — Light housekeeping — tidying living areas, vacuuming, mopping, dusting
- — Laundry — washing, drying, and folding client’s clothing and linens
- — Meal planning and preparation consistent with the client’s dietary needs and preferences
- — Grocery shopping or accompanying client to appointments as directed
- — Errand running as specified in the service plan
Documentation and Reporting
- — Document all services delivered in the client’s record accurately and on the date services are provided
- — Record and promptly report to the RN Supervisor any changes in the client’s condition, behavior, appearance, or home environment per 12VAC5 381-360(B)(4)
- — Complete visit notes, timesheets, and any required agency forms in a timely and accurate manner
- — Report any incidents, accidents, or concerns immediately to the RN Supervisor or on-call staff
- — Report any suspected abuse, neglect, or exploitation immediately per §63.2-1606 of the Code of Virginia
Service Plan Compliance
- — Review the client’s ISP with the RN Supervisor prior to the first visit and adhere to the plan at all times
- — Provide services only within the scope of duties specified in the ISP and this job description
- — Do not perform skilled nursing tasks, administer medications, or exceed the scope of non-skilled personal care
- — Notify the RN Supervisor of any client request for services outside the ISP
Professionalism and Client Relations
- — Arrive on time for every scheduled visit; follow call-out procedures when unable to report to work
- — Wear identification badge at all times while in a client’s home per 12VAC5-381-200(M) and POL-0610
- — Maintain strict confidentiality of all client information per HIPAA and organizational policy
- — Treat all clients with dignity, respect, and cultural sensitivity at all times
- — Respect client rights, including the right to refuse services, at all times per 12VAC5-381-230
- — Maintain a professional appearance consistent with the organization’s dress code
- — Maintain boundaries — do not accept gifts, money, or enter into personal financial arrangements with clients or their families
Minimum Qualifications
Per 12VAC5-381-290, each Home Attendant must meet one of the following six qualification pathways and must be able to speak, read, and write English:
- Current certification as a Nurse Aide (CNA) issued by the Virginia Board of Nursing
- Satisfactory completion of a nurse aide education program approved by the Virginia Board of Nursing
- Satisfactory completion of a nursing education program preparing for RN or LPN licensure
- Current enrollment in a nursing education program with at least one completed clinical nursing course involving direct client care
- Satisfactory passage of a competency evaluation program meeting the criteria of 42 CFR 484.36(b)
- Satisfactory completion of the DMAS Personal Care Aide Training Curriculum (2003 edition) — for personal care services only
Documentation of the applicable qualification pathway must be on file in the employee’s personnel file prior to independently providing services.
Additional Requirements
- — Minimum 18 years of age
- — High school diploma or GED preferred
- — Must pass Virginia criminal background check and signed sworn statement per §32.1-162.9:1
- — Must not be listed on the Virginia Nurse Aide Abuse Registry or any other state abuse registry
- — Current CPR / First Aid certification required (or obtained within 30 days of hire)
- — Valid Virginia driver’s license and reliable transportation required
- — Must complete all 9 topics of new employee orientation per 12VAC5-381-200(I) before providing services
Knowledge, Skills, and Abilities
- — Genuine compassion for elderly, disabled, and chronically ill individuals
- — Ability to follow written and oral instructions and work independently in a client’s home
- — Strong communication and interpersonal skills with clients, families, and supervisors
- — Reliability, punctuality, and commitment to consistent attendance
- — Cultural sensitivity and respect for diverse backgrounds, beliefs, and preferences
- — Ability to maintain professional boundaries in a home setting
Orientation and Ongoing Training
- — Must complete new employee orientation covering all 9 required topics per 12VAC5-381-200(I) prior to first client assignment
- — Must complete a minimum of twelve (12) hours of annual in-service training and education per 12VAC5-381- 360(G)
- — In-service training may be conducted in conjunction with on-site supervisory visits
- — Must maintain current CPR / First Aid certification at all times
- — Must participate in all required training as directed by the RN Supervisor or Administrator
Physical Requirements and Working Conditions
- — Frequent bending, kneeling, stooping, and reaching in client home environments
- — Ability to lift, transfer, and reposition clients — up to 50 lbs with or without assistive devices
- — Prolonged standing and walking throughout shift
- — Must be physically capable of performing all personal care tasks specified in client ISPs
- — Work is performed exclusively in clients’ private homes; conditions vary by client
- — Exposure to cleaning products, bodily fluids, and communicable illness is possible; use of PPE required
- — Must be able to travel reliably to client homes within the service area
