About This Position

General Summary:

The Case Manager will join Nurtury’s Family Services Department, reporting directly to the Director of Family Services. As a dedicated and skilled professional, the Case Manager will serve as the primary liaison for our priority populations, including DCF, young parents under 24, and families experiencing homelessness. In collaboration with Family Partners, the Case Manager will support individualized family needs, facilitate Individualized Family Plans, and conduct developmental evaluations. This role is ideal for someone passionate about making a positive impact in the community by supporting families and children, particularly those with high needs facing challenging circumstances.

Essential Functions:

  1. Primary liaison for our priority population such as: DCF, Young Parents Under 24, and Families Experiencing Homelessness.
  2. Participates in regular/quarterly meetings with DCF.
  3. Responsible for supporting the contractual needs of the children and families including supporting the individualized needs of each family such as referrals to community resources and DCF reporting.
  4. Responsible for facilitating Individualized Family Plans and developmental goal setting plans to understand each family's needs, support them in reaching their future goals, and monitor their progress towards success.
  5. Performs developmental evaluations such as: ASQ’s/ASQ-SE’s for newly enrolled and families as needed.
  6. Facilitates and supports parent workshops, education, self-sufficiency, and other related enrichment activities for priority population.
  7. Completes standard administrative and statistical reports as needed.
  8. Establishes and maintains relationships with appropriate service providers and community agencies.
  9. Provides families with ongoing support in navigating referral processes such as: Early Intervention, Public School Systems, Public Assistance Benefits, and/or Hospitals.
  10. Responsible for providing one-on-one assistance to identified high-needs families. (identified either through evaluations, center teachers, FCC providers, family)
  11. Perform other related duties as required.


Knowledge, skills and abilities required to perform the essential functions:

  • Bachelor’s degree in related field preferred, or relevant years of experience
  • Bi-lingual, written and verbal strongly preferred (Spanish/English)
  • Experience in one or more of the following:
    • working with families of underprivileged socio-economic backgrounds
    • working with multicultural populations
    • meeting diverse family needs, stabilizing challenging circumstances, & supporting children and families who have experienced trauma.
    • knowledge of services and resources available to families within the community
  • Strong knowledge of Early Childhood Education, Social Work, or related Human Service field required
  • Sensitivity to and experience with handling confidential information and interactions, interacting in a culturally appropriate and responsive manner with a wide range of individuals, and navigating within and across different teams and levels of an organization
  • Strong oral and written communication skills
  • Excellent organizational and interpersonal skills
  • Computer efficiency with MS Office, internet/e-mail/virtual meeting systems
  • Experience with child evaluation systems such as: ASQ/ASQ-SE
  • Some local travel required
