About This Position

Statement of Purpose:

Responsibilities include intake psychosocial assessments, outreach, supportive, educational, and concrete case management services to patients and families served by JMH practices. The social worker collaborates with JMH practice teams, most commonly the practice care manager, to assist patients according to above mentioned responsibilities. The social worker also provides consultation to the health care providers regarding patients' care needs and/or community resources.

MAJOR TASKS, DUTIES AND RESPONSIBILITIES

  1. Specific Responsibilities
    1. Administrative
      1. Participates in organizational meetings pertinent to the profession of Social Work.
      2. Participates in social work consultation about his/her work in accordance with plan development with supervisor and practice clinical providers.
      3. Supports practices in meeting organizationally identified quality measures.
      4. Participates in assigned committees.
    2. Clinical
      1. Performs psychosocial assessments of patients and families referred by care managers and providers. These patients include, but are not limited to; persons in need of resource assistance or protection; persons experiencing emotional/mental health crises; and patients with chronic medical diagnoses.
      2. Provides social work services and/or consultation in collaboration with care managers for patients with problems identified through psychosocial assessments. Services may include: crisis intervention; supportive short-term counseling; problem-solving; information and referral, respite care planning; end of life support; and assistance with environmental, transportation and/or legal concerns. Services are largely provided within the practice or by phone.
      3. Collaborates with other practice care providers in development and implementation of interdisciplinary care plans.
      4. Coordinates referrals to and acts as liaison to community agencies regarding services to individual patients in collaboration with care managers.
      5. Provides internal and external systems advocacy as indicated in care plans of individual patient in collaboration with the care managers.
      6. Maintains current knowledge of community resources relevant to the JMH patient population.
      7. Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence.
      8. Participates in relevant training and educational opportunities designed to enhance knowledge and skills.
      9. Provides social work consultation to other care providers regarding patient problems and community services.
      10. Documents all social work activities in the patients' medical record.
      11. Performs other duties as assigned.
    3. Teaching
  1. Provides ongoing training to care managers and providers regarding social work services and selected psychosocial problems and/or resources.

EDUCATION: The social worker must have a minimum of BSW from an accredited social work program.

EXPERIENCE: 3 years’ experience in a clinical setting and/or ambulatory care setting preferred.

CONTACTS: Excellent communication and team skills including ability to form strong collaborative interdisciplinary partnerships across care settings.

Sound computer knowledge and skills including an aptitude for using health information technology to guide care management activities and functions. Ability to work independently and meet deadlines. Creativity and strong organizational skills.

PHYSICAL DEMANDS: Sitting, standing, walking, extensive phone and keyboard usage.

WORKING CONDITIONS: Normal Office.
