About This Position

Gifford Health Care is seeking a Clinical Care Manager to join our grant-funded Community Health Team. In this role, you will provide care management for patients with high-risk or complex needs, helping them improve health outcomes, manage chronic conditions, and navigate the healthcare system.

Using motivational interviewing, behavior change strategies, and a patient-centered approach, the Clinical Care Manager partners with individuals to set meaningful health goals, develop actionable care plans, and build long-term self-management skills.

This position also supports patients in planning for changing needs related to aging, including assessing future care requirements, identifying community and in-home supports, understanding long-term care options, and addressing safety, mobility, and social needs. You will connect patients and families with aging services, home health agencies, and community partners to promote independence and enhance quality of life.

Working closely with providers, clinic staff, community partners, and the extended Community Health Team, this role helps coordinate and integrate patient care, ensuring continuity and access to appropriate services.

As part of the Vermont Blueprint for Health, the Clinical Care Manager contributes to prevention, care coordination, and statewide population health initiatives. Confidentiality and adherence to Gifford Health Care policies and procedures are essential.

Requirements:

  • Current Vermont license as a LPN, RN, OT, OTA. PT, or PTA
