Job Description
Provide daily support and activity implementation for individuals supported in the program
Minimum Qualifications:
Must be at least 18 years of age or older
Must have high school diploma or GED
Must have valid Driver’s License or ID and social security card or proof of eligibility to work
Must be able to clear finger print clearance
Must be able to pass pre-employment physical and TB test
Must be able to pass CPR/First Aid Certifications
Previous experience working with individuals with developmental disabilities preferred
Must complete Direct Support Professional (DSP) and Registered Behavior Technician (RBT) certification within 6 months of hire date.
Multilingual a Plus, Bilingual preferred
Comply with confidentiality and HIPAA requirements
Duties and Responsibilities:
Supervision of individuals participating in day program activities
Provide support in daily personal care and activities of daily living, including but not limited to: restroom support, medication administration assistance as needed
Assist in maintaining client records and documentation, including but not limited: to goal progress documentation, data collection
Assist in keeping facility clean by helping complete daily chores
Assist in preparing and serving meals such as snacks and lunch
Implement assigned activities using person-centered practices
Supervise individuals on outings and ensure participation using person-centered practices
Assist with transporting individuals on outings and to/from home (if applicable)
Assist with development and update of participant Individual Service Plans (ISP)
Implement Individual Service Plan (ISP) goals as directed
Assist in responding to medical emergencies
Report concerns about individuals or the program to a supervisor
Assist in completion of incident reports as needed
Assist in mentoring new staff as directed
Must comply with personal rights of individuals supported
Must act as a mandated reporter and report all suspicions of abuse to the appropriate authorities
Must comply with all Discovery Community Day Program policies and procedures
Must comply with all applicable local, state and federal regulations and laws
Must comply with CCLD Title 22 and DDS Title 17 regulations
Attend staff training and meetings as required
Complete all training requirements on a timely basis as required including but not limited to:
o Nonviolent Crisis Prevention & Intervention Training (CPI)
o Professional Assault Crisis Training (Pro-ACT)
o Ukeru Systems – Restraint free Crisis Intervention program certification
All other duties as assigned
