Transitional care pharmacists are an integral component of the multidisciplinary team that cares for a patient during their stay at Hendrick Medical Center. The pharmacist is the bridge between outpatient and inpatient care by facilitating a safe and effective transition into the hospital system or discharge to outpatient services.
