Front Office Care Coordinator Adult Foster Care

**Location:** 1214 Park St., Stoughton, MA (On-site only)

**Schedule:** Full-time, Monday–Friday, 9:00 AM–5:00 PM

**Compensation:** $50,000/year (salary, exempt) — with opportunity for renegotiation after 6 months based on performance

**Start Date:** Immediate

**Probationary Period:** 90 days (benefits begin after probation is successfully completed)

About the Role

We are seeking a Front Office Care Coordinator for our Adult Foster Care (AFC) program in Stoughton, MA. This role is the backbone of daily operations, ensuring that patients, staff, and providers are supported in every aspect of enrollment, compliance, payroll tracking, and facility management. The ideal candidate is pleasant, professional, detail-oriented, highly organized, and multilingual when possible.

Key ResponsibilitiesPatient & Provider Communication

- Welcome patients and families warmly; use professional, empathetic communication.
- Contact doctors, case managers, and social workers to ensure records and authorizations are submitted on time.
- Support the enrollment process by helping families complete documentation quickly and accurately.
- Stay on top of provider and patient follow-ups to prevent enrollment delays.

Front Office & Clinical Support

- Provide administrative support to the Program Director

- Create and maintain detailed schedules for nurses, case managers, and social workers.
- Prepare and distribute packages, notices, and binders (including those that must be hand-delivered to patients’ homes).
- Assist with marketing and outreach materials such as flyers and program information packets.

Compliance & Licensing

- Perform CORI checks, OIG exclusion checks, and other required screenings for all staff.
- Track and update staff licensing, credentials, and training renewals to ensure compliance.
- Maintain facility binders and safety logs (e.g., fire extinguisher checks).
- Ensure compliance with MassHealth system requirements and maintain readiness for surveys and audits.

Records & Data Entry

- Process participant intake forms, PCP orders, care plans, and recertifications.
- Scan, upload, and organize records in EHR systems.
- Maintain accurate electronic and physical binders.
- Build and update Excel spreadsheets for admissions, compliance, licensing renewals, and safety.
- Generate weekly/monthly reports for management.

Payroll & Billing Support

- Prepare payroll trackers to ensure accurate hours and caregiver records.
- Provide trackers to the biller for timely submission.
- Verify payroll documentation for accuracy before submission.
- Assist with payroll packet collection and compliance review.

Required Qualifications

- 2–3+ years in a medical office or healthcare compliance/admin role.
- Experience preparing payroll trackers and managing HR spreadsheets.
- Knowledge of CORI/OIG checks, staff licensing, and MassHealth compliance.
- Proficiency in Microsoft Excel and Word.
- Strong ability to prioritize and stay on top of multiple deadlines.
- Extremely pleasant, professional communication skills with patients, families, and providers.
- Ability to learn and manage EHR, and MassHealth systems quickly.
- Familiarity with HIPAA and PHI protections.

Preferred Qualifications

- Bilingual or multilingual strongly preferred: English with Spanish, Haitian Creole, Portuguese, Vietnamese, or Chinese (Mandarin or Cantonese).
- Experience in AFC, Adult Day Health, or community-based long-term care programs.
- Background in healthcare marketing, outreach, or patient enrollment.

Compensation & Benefits

- Salary: $50,000/year — renegotiable after 6 months based on performance.
