About the Role:

The Licensed Practical Nurse (LPN) at Elwood Skilled Nursing Facility (SNF) plays a critical role in delivering high-quality nursing care to residents in a long-term care environment. This position involves providing direct resident care, monitoring patient health, and assisting with the implementation of individualized care plans under the supervision of registered nurses and physicians. Minimum Qualifications:

Responsibilities:
