Location: Salt Lake, Utah (Area)

RN Case Manager Requirements:

  • Reliable transportation

RN Case Manager Responsibilities:

  • Coordinate the overall care of the patient while interacting with patient, family, physicians, Interdisciplinary Team members and other healthcare professionals.
  • Provide professional nursing care for both home health and hospice patients in their place of residence.
  • Complete initial, comprehensive and ongoing assessments for patients to determine their needs.
  • Conduct nursing visits for pain and symptom management, wound care, medication administration and other skilled care as ordered by the physician.
  • Complete all documentation and charting accurately and in a timely manner.
  • Monitor and supervise those involved ensuring that the patient receives the best possible care.

Schedule:
Full-Time flexible; Monday- Friday expected; 8-5 flexible
On-call not required, unless desired (we have a seperate on-call team)

Pay:
DOE

Licenses Required:
  • Active Utah RN license
  • Current CPR certificate
  • Valid Utah driver license with car insurance
