Pay: $22–$25/hour for the following counties Placer, Sonoma, Marin, Napa, Butte,
Job Type: Full-Time (with Benefits)
Location: Northern California – up to 50-mile driving radius
About the Role:
We’re seeking compassionate, reliable individuals to join our Enhanced Care Management (ECM) outreach team, supporting patients enrolled through Partnership HealthPlan of California. This is an “Uber Eats meets Home Health” model — bringing critical support directly to the homes of patients who cannot be reached by phone or email.
As an In-Person Welfare Check Specialist, you will conduct home visits to check on patients’ well-being, connect them with their Lead Care Manager using a company-issued phone, and occasionally pick up medications from Federally Qualified Health Centers (FQHCs) to deliver to patients as needed.
Key Responsibilities:
Conduct in-person welfare checks for patients enrolled in ECM who are unresponsive to phone or email outreach
Drive to patient homes (up to 50 miles one-way)
Use a company phone to connect patients with their Lead Care Manager during the visit
Pick up prescribed medications from FQHC clinics and deliver them to patients securely
Provide compassionate, culturally sensitive support during each visit
Document outcomes and observations in real time using a mobile system
Qualifications:
Bilingual in English and Spanish required
Valid driver’s license and excellent driving record
Reliable personal vehicle (mileage reimbursement provided if applicable)
Strong communication and people skills
Comfortable using smartphones and basic mobile apps
Ability to work independently and manage a dynamic field-based schedule
Passion for improving care access for vulnerable populations
