Job Description

Case ManagerPosition Summary

Under the direction of the Resident Manager, the Case Manager is responsible for supporting program operations while utilizing Psychiatric Rehabilitation principles to promote recovery, community integration, independence, and an improved quality of life for residents. This role focuses on helping individuals achieve their personal recovery goals through collaboration, advocacy, skill development, and supportive services.

The Case Manager develops meaningful relationships with residents and works closely with team members, family members, significant others, and community resources to foster a safe, supportive, and recovery-oriented environment. This position serves as a key resource in promoting the Psychiatric Rehabilitation Model and supporting its day-to-day implementation.

Key ResponsibilitiesRecovery and Rehabilitation Services
  • Utilize Psychiatric Rehabilitation principles to support resident recovery, community integration, and overall wellness.
  • Assist residents in identifying, developing, and achieving individualized recovery goals.
  • Support residents in maintaining physical health, mental health, and emotional well-being.
  • Encourage active participation in the recovery process and self-directed goal setting.
  • Promote independence, self-help skills, and personal responsibility.
  • Facilitate opportunities for social inclusion and meaningful community participation.
Resident Support and Engagement
  • Build trusting, supportive, and professional relationships with residents.
  • Provide emotional support while maintaining appropriate professional boundaries.
  • Maintain a safe, welcoming, and recovery-focused living environment.
  • Assist residents with daily living skills and instrumental activities of daily living (IADLs), including education and skill-building.
  • Encourage the development of natural support systems within the community and personal networks.
  • Foster a collaborative and supportive atmosphere within the residence.
Advocacy and Care Coordination
  • Advocate for resident rights, preferences, and recovery goals.
  • Empower residents to develop self-advocacy skills and participate actively in decision-making.
  • Collaborate with family members, treatment providers, community partners, and other support networks.
  • Assist team members and service providers in understanding and applying Psychiatric Rehabilitation principles.
  • Coordinate services and resources to support individualized recovery plans.
Program Operations
  • Assist in ensuring all program operational procedures are followed.
  • Maintain accurate and timely documentation in accordance with organizational policies and regulatory requirements.
  • Participate in team meetings, trainings, and professional development activities.
  • Uphold the values, mission, and policies of the organization in all aspects of service delivery.
  • Perform other duties as assigned to support program operations and resident needs.
Household Support
  • Perform light housekeeping duties as needed.
  • Support residents in maintaining a clean, safe, and organized living environment.
  • Educate residents on daily living skills and household management techniques.
Required QualificationsEducation
  • Bachelor’s Degree in a Human Services discipline or related field.
Licensure
  • Valid Driver’s License with an acceptable driving record.
Preferred Skills and Experience
  • Experience working with individuals experiencing mental health challenges, behavioral health needs, or psychosocial rehabilitation services.
  • Knowledge of Psychiatric Rehabilitation and Recovery-Oriented practices.
  • Experience providing community-based, residential, or case management services.

Physical Requirements

This position is classified as Medium Work and requires:

  • Exerting up to 50 pounds of force occasionally.
  • Exerting up to 20 pounds of force frequently.
  • Exerting up to 10 pounds of force constantly to move objects.

The employee must be able to:

  • Stand and sit for extended periods.
  • Walk throughout the facility and community settings.
  • Use hands and fingers to handle, operate, and manipulate objects.
  • Climb stairs and navigate residential environments.
  • Communicate effectively verbally and in writing.
  • Observe and assess resident needs and environmental conditions.
  • Push and pull objects as necessary.
  • Perform repetitive motions.
  • Reach above and below shoulder level.
  • Kneel, stoop, crouch, and crawl when required.
Working Conditions

The Case Manager may work in residential, office, and community-based environments and may be exposed to:

  • Dust, fumes, mists, and gases
  • Extreme heat or cold
  • Wet and humid conditions
  • Loud noise levels
  • Confined spaces
  • Mechanical hazards
  • Chemical hazards
  • Electrical hazards
  • Burn hazards

The position also requires community travel and interaction with residents in a variety of settings.
