QUALIFICATIONS:

Associate’s Degree or sixty (60) credit hours and four (4) years experience working with persons with intellectual disabilities, Bachelor’s Degree with (2) years experience working with individuals with intellectual disabilities, or Master’s Degree and one (1) years experience working directly with persons with intellectual disabilities. Driver’s License and use of own personal vehicle required; Must have a Driver's License and use of a vehilcle; Excellent clinical, communication and interpersonal skills, excellent writing skills, thorough knowledge of Intellectual Disabilities Field and Best Practices.

JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by managing an effective team to provide quality services. Responsible for the overall leadership and supervision of PATH’s Community Living Arrangements Sites in accordance with PATH’s mission values, and principles.

SPECIFIC DUTIES:

  1. Provide supervision of assigned residential sites
  2. Maintain and enforce compliance with DPW/ODP 6400 regulations
  3. Conduct Site Orientation of new staff to assure that staff are knowledgeable about residents needs/services
  4. Regularly observe and provide feedback/training to Residential Skills Trainers to ensure that their interactions with each resident are consistent with the individual’s goals, uses techniques and approaches that positively supports individual’s ISP, as well as ODP’s Everyday Lives Philosophy
  5. Management of all site related issues: support of residents, ensure cleanliness and orderliness of sites, regular employee feedback, evaluations and when necessary, trainings.
  6. Monitor assigned residential sites to ensure health and safety of residents and inform PATH’s Facilities Department of any facility problems in a timely manner.
  7. In cooperation with Community Inclusion Manager, ensure opportunities for residents to engage in community integrated activities in keeping with ISP goals and resident preferences.
  8. Provide direct services as needed, especially during staff vacancies in consultation with the Scheduling Specialist
  9. Attend and facilitate monthly meetings: site meetings and Support Coordinator Monitoring
  10. Participate in the provision of 24 hour /7 day a week On Call supervisory coverage on a scheduled basis which includes responding to phone calls in accordance with PATH On-Call Policy and Procedures and providing face to face intervention as indicated to resolve a crisis
