Job Description:
Join our dynamic team! Are you ready to make a difference where it matters most? Are you ready to lead change and support second chances? You will play a critical role in supporting, justice-involved individuals with behavior health and medical needs as they transition from incarceration to community life. You will work collaboratively with the Sheriff’s Office and the Sheriff’s Office’s contracted medical provider to screen and assess the psychiatric, medical, social services, employment, and residential needs upon entry into the Chesapeake Correctional Center, (CCC.)

You will be responsible for coordinating pre-release and post-release case management for CCC inmates with mental health, critical care medical, and substance use disorder needs to ensure a smooth transition to the community and services. Provide in community support for at least 30 days after release to ensure continuity of care and reduce recidivism. Create a discharge plan with goals that reflect the needs of the inmate.

You willcollaborate and coordinate with Chesapeake Integrated Behavioral Health’s community-based services and community partners to refer and link inmates with resources to support community re-entry, to include transitional housing or other housing support, and minimize recidivism.

The discharge plan examples are assisting inmates with a SMI resident with obtaining photo identification, birth certificates, apply for Medicaid and/or GAP applications/reinstatement, apply for Social Security disability/SSI assistance, assist with emergency food and clothing as approved through the Inmate Jail Program Fund, etc.


Required Qualifications:
VOCATIONAL/EDUCATIONAL REQUIREMENT:
Requires a bachelor’s degree in social work, psychology, closely related field, or another degree as approved by the Virginia Department of Health Professions. A master’s degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.

EXPERIENCE REQUIREMENT:
In addition to satisfying the vocational/education standards, this position requires a minimum of five years of related, full-time equivalent experience. Depending on assignment, may require Qualified Mental Health Professional (QMHP – Adult or Child) or Qualified Developmental Disability Professional (QDDP) certification and highest Part C standards within 6 months of hire; CPR, First Aid and medication administration certifications within 3 months of hire, Certified Substance Abuse Counselor (CSAC) certification upon hire, Case Management Modules and Intellectual Disabilities Workbook completed within thirty days of hire, Part C Early Intervention Certification within six months of hire, and/or Pre-Screening Certification within 6 weeks of hire (12 weeks for part-time staff).

SPECIAL REQUIREMENT:
Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.


Preferred Qualifications:
Minimum of five years of experience required.
Experience with case management, reentry planning or community mental health is a plus. Strong communication skills and a deep sense of empathy.
