Sangre de Cristo Community Care is seeking an experienced Clinical Manager to lead and support our growing Home Health team in Pueblo, Colorado. The Clinical Manager is responsible for overseeing the day-to-day clinical operations of our Pueblo Home Health program. This role provides leadership, supervision, and support to clinical staff while ensuring compliance with regulatory requirements, accreditation standards, and organizational policies.
The Clinical Manager partners closely with physicians, nurses, therapists, and other healthcare professionals to coordinate high-quality patient care and support continued program growth. This position plays a key role in staff development, quality improvement, patient satisfaction, and operational success.
Essential Responsibilities- Provide leadership, supervision, and support to home health clinical staff.
- Ensure the delivery of high-quality, patient-centered care in accordance with agency standards and regulatory requirements.
- Monitor clinical documentation, patient care plans, and quality outcomes.
- Ensure compliance with Medicare Conditions of Participation, state regulations, accreditation standards, and agency policies.
- Assist with staff recruitment, onboarding, coaching, performance management, and professional development.
- Collaborate with physicians, referral sources, and interdisciplinary team members to ensure seamless care coordination.
- Support quality improvement initiatives and performance improvement activities.
- Manage clinical resources effectively to promote operational efficiency and excellent patient care.
- Participate in strategic growth initiatives to expand home health services throughout the Pueblo community.
- Serve as a clinical resource and mentor, fostering a culture of accountability, teamwork, and excellence.

Requirements: Required- Registered Nurse (RN), Physical Therapist (PT), or Speech Therapist (ST) with current Colorado licensure in good standing.
- Minimum of two (2) years of management or supervisory experience in a healthcare setting.
- Home health experience strongly preferred.
- Current CPR certification.
- Strong leadership, communication, organizational, and problem-solving skills.
- Ability to work collaboratively with interdisciplinary teams and diverse populations.
- Flexibility to provide leadership support after hours, on weekends, and participate in an on-call rotation.
- Valid driver's license, reliable transportation, and proof of adequate automobile insurance.
- Bachelor's Degree in Nursing (BSN) or related healthcare field.
- Previous home health leadership experience.
- Knowledge of Medicare regulations, OASIS, and home health quality measures.
