Memory Care Manager (LPN) - Assisted Living Facility

Elan Assisted Living is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team as a Memory Care Manager. Key Responsibilities:

- Oversee and coordinate daily operations of the memory care unit, ensuring high-quality resident care

- Develop, implement, and review individualized care plans tailored to residents' specific needs

- Supervise and support nursing staff, providing guidance and mentorship

- Monitor residents' health status, administer medications, and respond to medical emergencies

- Ensure compliance with all state and federal regulations, as well as facility policies

- Collaborate with families, physicians, and interdisciplinary team members to optimize resident well-being

- Maintain accurate documentation of resident care and facility activities

- Promote a safe, respectful, and engaging environment for residents and staff

Skills and Qualifications:

- Valid and current Licensed Practical Nurse (LPN) license

- Prior experience in memory care or geriatric nursing preferred

- Strong understanding of dementia and other memory-related conditions

- Excellent communication and interpersonal skills

- Ability to lead and motivate a team

- Compassionate, patient, and attentive to residents' needs

- Knowledge of healthcare regulations and compliance standards

- Ability to handle sensitive situations with professionalism and discretion
