Discharge Planner in Thibodaux, Louisiana at Thibodaux Regional Health System

Responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating physical and psychological factors, financial, social, and community resources with a focus on social issues/discharge planning under the supervision of the RN Case Manager for patients of TRHS.

Educational and/or Professional Qualifications:

One of the following is required:

• At a minimum an Associates degree in a healthcare related field or practical nursing.
• a minimum of 4 years’ experience in a healthcare related setting in lieu of education
• a minimum of 2 years’ experience working as a discharge planner.

Previous Experience:

1. Ability to prioritize responsibilities and use appropriate time management skills.

Licenses/Certifications:

**if Bachelors degree is in Social Work, must have license as a Registered Social Worker by the State of Louisiana.

**if Practical Nurse degree, must have license as a LPN in the State of Louisiana.

If degree is in other field, no license is required.
