We are expanding into Longview, Texas, and are seeking an experienced and motivated Physical Therapist to help establish and grow our rehabilitation program.
Position Overview
The Physical Therapist will evaluate, diagnose, and treat patients with musculoskeletal, orthopedic, neurological, and injury-related conditions. The PT will work closely with Axon providers and clinical staff to develop individualized treatment plans and ensure patients receive appropriate, well-documented care.
The ideal candidate is clinically strong, patient-focused, organized, and interested in being part of a growing organization.
Key Responsibilities
Perform comprehensive physical therapy evaluations and examinations.
Develop individualized treatment plans based on patient diagnoses, functional limitations, and goals.
Provide evidence-based therapeutic interventions.
Treat orthopedic, spine, musculoskeletal, and injury-related conditions.
Perform appropriate neurological and functional assessments.
Provide vestibular rehabilitation when appropriately trained.
Monitor patient progress and modify treatment plans as clinically indicated.
Document evaluations, daily treatment notes, progress reports, and discharge summaries accurately and timely.
Maintain documentation in accordance with company, clinical, and regulatory requirements.
Communicate effectively with NPs, physicians, PTAs, MAs, patients, and management.
Educate patients regarding exercises, home programs, injury prevention, and recovery expectations.
Maintain a safe, clean, and professional therapy environment.
Participate in team huddles and interdisciplinary communication.
Maintain appropriate patient records and support documentation necessary for personal-injury cases.
Maintain professional communication with referring offices and other healthcare professionals when appropriate.
Follow HIPAA, OSHA, safety, and company policies.
Contribute to the development and growth of the Longview rehabilitation department.
Qualifications
Required:
Doctor of Physical Therapy (DPT) or equivalent qualifying degree.
Current Texas Physical Therapy license.
Strong clinical assessment and treatment skills.
Excellent communication and interpersonal skills.
Strong documentation and organizational skills.
Ability to work independently and as part of a multidisciplinary team.
Preferred:
Experience in Orthopedics, Neurology, Personal Injury, or outpatient rehabilitation.
Vestibular rehabilitation experience or certification.
TBI rehabilitation experience.
Bilingual English/Spanish.
Experience working in a fast-paced outpatient environment.
