Occupational Therapist – Home Modification As in at Alta Healthcare Services

Alta Healthcare Services is seeking a Maryland-licensed Occupational Therapist to conduct in-home environmental assessments for Medicaid clients who require home modifications.

Responsibilities:

• Evaluate clients’ functional and accessibility needs.
• Assess home safety, mobility barriers, and fall risks.
• Recommend medically necessary modifications and adaptive equipment.
• Complete clear, timely Medicaid-compliant assessment reports.
• Coordinate with clients, caregivers, contractors, and care teams as needed.

Qualifications:

• Active Maryland Occupational Therapist license.
• Experience with home safety, accessibility, aging in place, or disability assessments.
• Strong documentation and report-writing skills.
• Reliable transportation for home visits.

Position: Independent Contractor | Flexible Schedule | Per-Assessment Compensation
Service Area: Montgomery County and surrounding Maryland counties
