Job Description
Registered Nurse (RN)
$65 - $67 an hour
Akicita Federal is hiring a Registered Nurse (Nurse Care Manager) to provide direct patient nursing care and care coordination services at the Pawnee Indian Health Center in Pawnee, OK. In this role you will serve a full continuum of patient needs — from in-clinic visits to community and home-based care — as part of an integrated primary care team.
Job Description-
- Provide direct patient nursing care within the scope of RN licensure on a daily basis.
- Manage care for empaneled patients assigned to a designated care team, including pre-visit planning, visit coordination, and post-visit follow-up.
- Serve as the primary clinical liaison between referral hospitals, treatment facilities, and patients and families.
- Assess patients' clinical, psychosocial, and functional status in collaboration with the care team and assist in developing plans of care.
- Conduct intake assessments, screening, and follow-up interviews with clients and family members.
- Provide patient education, adherence strategies, risk reduction counseling, and goal-setting support.
- Coordinate available resources to meet patients' needs and support clinical outcomes across homes, schools, clinics, job sites, and other community locations.
- Chart and document all care in the Electronic Health Record system or approved paper forms in accordance with Joint Commission standards, CDC guidelines, and Association for Infection Control guidelines.
Required
- Active, unrestricted Registered Nurse (RN) licensure.
- Current BLS
- Current Immunization records
- At least 2 RN experience
- Must be a US Citizen
