Registered Nurse Manager - Transitional Care Unit - The Lutheran Home Skilled Nursing Community
$95,000-$113,000/year, based on experience. Full-time day shift, with weekend on-call rotation. Lead care coordination during one of the most critical points in a resident's recovery.
The Lutheran Home is a faith-based skilled nursing community in Arlington Heights. As RN Manager of the Transitional Care Unit, you'll be the primary clinical coordinator for residents moving into and out of long-term or transitional care, working to prevent rehospitalizations and support successful discharges.
What you'll do
- Serve as the primary clinical coordinator for residents transitioning into and out of long-term or transitional care
- Oversee clinical care plans to support recovery and successful discharge outcomes
- Collaborate with physicians, therapy, case management, social services, and the interdisciplinary team
- Monitor resident progress and proactively address changes in condition to prevent rehospitalizations
- Educate residents, families, and staff on care plans and recovery goals
- Support admissions, assessments, and care coordination for short-term rehabilitation residents
- Ensure compliance with federal, state, and local regulations and organizational policies
- Participate in weekend on-call rotation to support continuity of care
What you'll need
Required:
- Active Illinois RN license
- Must be 18 years of age or older
- 2+ years of nursing experience in skilled nursing, transitional care, rehabilitation, or long-term care
- Experience with clinical assessments, care planning, and coordinating with interdisciplinary teams
Preferred:
- Prior leadership, care coordination, or nurse manager experience
- Strong clinical judgment and the ability to manage multiple priorities independently
