Service Coordinator
Salary: $60,000
Pathways to Housing DC (Pathways DC) is based in Washington, DC and serves people in the District and Montgomery County, MD. We are an innovative and nationally recognized nonprofit committed to ending homelessness for individuals with complex health challenges. Pathways DC is one of the originators of the “Housing First” model to end homelessness, in which services are provided without conditions and pre-housing requirements such as curfews, mandated sobriety or compliance with medication. We believe housing is healthcare and that housing is a basic human right. Pathways provides an array of services including housing, street outreach, case management, drop-in services, and behavioral health support. Our model is based on client choice and the idea that the people we serve are the architects of their recovery!
Service Coordinators are members of the Assertive Community Treatment (ACT) team who provide flexible, community based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals who have a mental illness and abuse substances .The Service Coordinator (SC) maintains a primary caseload of 10-12 clients and in collaboration with the transdisciplinary team coordinates and provides ACT services for all clients on the team in a compassionate and responsible manner.
RESPONSIBILITIES:
- Collaborate with client in the formulation of an Individual Recovery Plan (IRP) that is reviewed and modified with client on a regular basis;
- Provide outreach, case management, counseling, advocacy, and other needed services to clients in any environment including: the streets, shelters, jails, hospitals, apartments, office, etc; 80% of contact with clients is conducted in the field;
- Provide individually tailored services to each client in the following areas: housing, school, training & work opportunities, activities of daily living, health, wellness self-management, relapse prevention, money management & entitlements, medication support, self help & empowerment, problem solving, family life & social relationships and integrated treatment for substance abuse;
- Office Triage, maintain written and computerized records compile reports and complete other program documentation (e;g;, monthly notes, GAIN-SS, progress notes, incident reports, on-call logs, letters, psychosocial assessments); and other duties as deemed necessary.
- 80% QI Pass Rate
- Coordinate and monitor referrals to community services and advocate client participation;
- Lead groups, workshops, and in-services on topics relevant to identified areas of client need;
- Perform related work as assigned;
QUALIFICATIONS:
- Bachelors and 2 years’ experience in mental health services/human services; Or 3 years related experience; Or lived experience, some college/continued education and 1year experience paid or unpaid
- Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations;
- Career experience in social services, supported housing, or with persons with psychiatric disabilities;
- Excellent communication, writing, and computer skills;
- Bilingual English/Spanish a plus;
- Drivers License required;
