Job Description
Ability360 is seeking a detail-oriented and compassionate DDD Home Care Program Supervisor for the West Valley area. The office is located at 5025 E. Washington St. Suite 200, Phoenix, Z 85034. This position oversees consumer intakes and assessments, match and monitor Personal Care Assistants (PCAs), resolve conflicts, and ensure compliance with AHCCCS and Electronic Visit Verification (EVV) requirements.

Our Home Care program offers high-quality, person-centered, non-medical home care services to Arizona Department of Developmental Disabilities (DDD) members. Services may include attendant care, personal care, homemaker services, habilitation, and respite.
Essential Responsibilities
- Complete consumer intake and assessments to determine service needs, including type, hours, and schedules.
- Conduct complete supervisory monitoring visits as per the Arizona Health Care Cost Containment System (AHCCCS) requirements.
- Schedule Personal Care Assistants (PCAs) and applicants for all necessary DDD trainings.
- Match and refer qualified PCAs to consumers based on service needs and skills.
- Monitor PCA performance, competency, and professional conduct.
- Monitor PCA compliance with EVV and assist PCAs with resolving payroll concerns.
- Complete and maintain accurate documentation.
- Communication with Support Coordinators.
Qualifications
- 3+ years’ combined education/experience in case management or home care supervision.
- Knowledge of EVV systems and DDD.
- Bachelor’s Degree preferred.
- Have or be eligible for a Level 1 Fingerprint Clearance Card, a driver’s license, a clean driving record, and reliable transportation.
- Bilingual a plus.
- Strong problem-solving and conflict resolution skills.
- Ability360 to manage multiple priorities and deadlines.
- Strong written and verbal communication skills.
Work Environment
Forty percent of work time is spent in an office setting (Central Phoenix), while sixty percent is in the community (West Valley).
Schedule
Monday–Friday, 8:00 AM – 5:00 PM, with occasional evenings/weekends.
Salary
A competitive salary commensurate with experience with a complete benefits package including health, dental, vision, 403(b) retirement plan with employer match, and mileage reimbursement.
