PRN Certified Occupational Therapy Assistant (COTA) – Home Health
Position Summary
The PRN Certified Occupational Therapy Assistant (COTA) provides skilled occupational therapy services to patients in their homes under the direction and supervision of a licensed Occupational Therapist (OT) and in accordance with physician orders, the established plan of care, agency policies, and applicable state and federal regulations. The COTA assists patients in improving their ability to perform activities of daily living (ADLs), instrumental activities of daily living (IADLs), functional mobility, and overall independence while promoting safety and quality of life.
Essential Duties and Responsibilities
· Provide skilled occupational therapy interventions as directed by the supervising Occupational Therapist and outlined in the patient's plan of care.
· Implement therapeutic activities and exercises designed to improve strength, coordination, endurance, balance, fine motor skills, range of motion, and functional performance.
· Assist patients in developing or regaining the skills necessary to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
· Educate patients and caregivers on home exercise programs, energy conservation techniques, adaptive equipment, joint protection, and home safety strategies.
· Reinforce recommendations regarding the use of adaptive equipment and environmental modifications to maximize patient safety and independence.
· Observe, document, and report patient responses to treatment, progress toward goals, and changes in condition to the supervising Occupational Therapist.
· Complete treatment documentation accurately and within agency-established timeframes in accordance with Medicare guidelines and regulatory requirements.
· Communicate regularly with the supervising Occupational Therapist regarding patient status and the effectiveness of interventions.
· Participate in interdisciplinary team meetings and case conferences as requested.
· Maintain compliance with Medicare Conditions of Participation, HIPAA regulations, infection control standards, and agency policies.
· Maintain professional competency through continuing education and adherence to ethical standards.
· Accept PRN patient assignments based on agency staffing needs and availability.
Qualifications
· Graduate of an accredited Occupational Therapy Assistant program.
· Current, unrestricted Certified Occupational Therapy Assistant (COTA) license or certification as required by the state of practice.
· Current CPR/BLS certification.
· Valid driver's license, reliable transportation, and current automobile insurance.
· Minimum of one year of clinical experience preferred; home health experience preferred.
· Knowledge of Medicare home health regulations and documentation requirements is preferred.
· Strong communication, organizational, and time management skills.
· Ability to work independently while maintaining regular communication with the supervising Occupational Therapist.
Physical Requirements
· Ability to travel independently to patients' homes throughout the assigned service area.
· Ability to lift, transfer, and assist patients as needed (up to 50 pounds with or without reasonable accommodation).
· Ability to stand, walk, bend, kneel, stoop, reach, and perform physical activities necessary to provide patient care.
· Ability to safely operate a motor vehicle and transport therapy supplies or adaptive equipment as needed.
Supervision
The Certified Occupational Therapy Assistant practices under the supervision of a licensed Occupational Therapist and provides services only as permitted by state practice acts, Medicare regulations, and agency policies. The COTA does not perform initial evaluations, establish or modify plans of care, or complete discharge assessments unless permitted by applicable law and regulation.
Work Schedule
This is a PRN (as-needed) position. Hours are not guaranteed and are based on patient census and agency staffing needs. The PRN Certified Occupational Therapy Assistant is expected to maintain flexibility and communicate availability for patient assignments.
