As a Family Model Provider, you will welcome an individual into your home and provide a safe, supportive, family-like environment while helping them achieve greater independence, dignity, and community inclusion. Responsibilities- Provide a safe, stable, and welcoming home environment.
- Assist with daily living skills and personal growth.
- Support community participation and social activities.
- Encourage independence, self-determination, and personal choice.
- Assist with transportation to appointments, activities, and community events.
- Coordinate with PDI staff, nursing staff, and support teams.
- Maintain required documentation and participate in training.
- Ongoing training and support.
- 24/7 on-call assistance.
- Access to nursing and professional support services.

Requirements:- Must be at least 21 years old.
- Have a valid driver's license and acceptable driving record.
- Pass all required background checks and drug test.
- Have a suitable home that meets state licensing requirements.
- Ability to provide compassionate, person-centered support.
- Previous experience in caregiving, healthcare, education, social services, foster care, or developmental disabilities is preferred but not required.
