Description: BASIC PURPOSE OF THE JOB

The Clinical Manager is responsible for overseeing and ensuring the delivery of high-quality patient care across all home health service lines, including nursing, physical therapy, occupational therapy, and speech therapy. The Clinical Manager will coordinate multidisciplinary care, supervise clinical staff, ensure compliance with regulatory requirements, and implement quality improvement initiatives to enhance patient outcomes. This position requires a licensed Occupational Therapist (OT), Physical Therapist (PT), Speech-Language Pathologist (SLP), or Registered Nurse (RN) with demonstrated leadership experience and expertise in pediatric care. Preferred Home Health experience.

JOB REQUIREMENTS

Supervisory Responsibilities: YES

Minimum Education: Bachelors Degree Required; Masters degree preferred

Degree: Clinical Based Degree required (RN, PT, OT, SLP)

License/Certification Required: TN or Compact Licensure required. BLS required.

Minimum Work Experience: 3-5 years of Clinical experience required. Management/supervisory experience preferred. Must be a licensed, insured driver.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
  • Graduate from an accredited school of professional nursing, PT, OT or SLP.
  • Comfortable working knowledge of nursing process, HME, home infusion, rehabilitation services and home infusion.
  • Demonstrates professional judgment, outstanding communication skills, capable of multitasking, attentive to detail, highly organized, flexible and able to function autonomously.
  • Must have a valid Tennessee Drivers License.
DUTIES AND RESPONSIBILITIES
  • Collects and ensures the collection of comprehensive data pertinent to the healthcare needs of patients across all home health service lines.
  • Ensures that staff across all service lines complete safety and risk assessments accurately and in a timely manner.
  • Assist staff in developing individualized care plans that are culturally, environmentally, and age appropriate.
  • Ensures continuity of care within the plan and alignment with current home health practice standards.
  • Facilitates integration and coordination of care across all service lines and disciplines.
  • Systematically evaluates the quality and effectiveness care in all service lines. Participates in activities to assess the quality of care as appropriate. These activities may include but are not limited to: Identification of aspects of care that affect quality. Analysis of quality data to identify opportunities for improvement of care. Development of policies, procedures, and practice guidelines reflective of quality care. Identification of indicators used to monitor quality and affect care. Ongoing data collection for monitoring the quality and effectiveness of care. Articulation of recommendations for quality improvement. Lead interdisciplinary teams to implement activities and evaluate clinical practice or health issues.
  • Evaluates patient care and outcomes through routine observation and supervisory home visits.
  • Ensures that care plans are consistently updated to reflect changes in patient condition and treatment goals.
  • Leads interdisciplinary teams to implement activities and evaluate clinical practices.
  • Develops and implements quality improvement initiatives using data analysis and evidence-based practices.
  • Identifies opportunities to improve care delivery and patient outcomes through policy and procedure development.
  • Provides leadership and guidance to all clinical staff across service lines.
  • Holds staff accountable for performance, compliance, and professional behavior.
  • Directly supervises clinical staff and ensures consistent performance evaluation and feedback.
  • Acts as a resource for staff in complex clinical situations and supports professional development.
  • Encourages teamwork, problem-solving, and innovation among staff.
  • Facilitates collaboration between disciplines and ensures consistent communication regarding patient care.
  • Serves as a mentor and fosters a culture of accountability and excellence.
  • Develops and manages the department budget, ensuring effective use of resources and cost-efficient care delivery.
  • Monitors financial performance, identifies variances, and implements corrective action as needed.
  • Works with leadership to develop strategic plans and allocate resources based on patient care needs and department goals.
  • Identifies and secures necessary resources to meet patient care needs while maintaining financial accountability.
  • Ensures that staff communicate effectively with patients and families regarding the care plan and expectations.
  • Ensures that all staff adhere to state, federal, and agency standards and regulations.
  • Develops and maintains policies and procedures that align with current home health regulations and best practices.
  • Identifies and secures necessary resources to meet patient care needs.
  • Represents the Home Health Department in hospital leadership meetings and strategy sessions.
  • Serves as the key liaison between home health and hospital leadership to align goals and care strategies.
  • Participates in hospital committees to provide insight on home health services and improve care coordination.
  • Develops and maintains professional relationships with hospital providers, including physicians, nurses, and therapists, to facilitate seamless care transitions from hospital to home.
  • Identifies gaps in care and collaborates with hospital teams to develop solutions that enhance patient care continuity.
  • Engages in professional networking and community outreach to promote home health services and build partnerships with external healthcare providers.
  • Utilizes hospital resources and time respectfully and accountability.
  • Willingly accepts any other assignment that may be requested
PHYSICAL REQUIREMENTS
  • Note: Reasonable accommodations may be made for individuals with disabilities to perform the essential functions of this position.
  • Light lifting, pushing and pulling is required for 10-20 lbs occasionally and frequent moving of objects of less than 10 lbs is required. Frequent sitting with some walking, standing, squatting, bending and reaching is required. Keyboard/computer use and/or repetitive motions may be required.
  • Heavy lifting, pushing, and pulling is required up to 50 lbs occasionally, 20-30 lbs frequently, and 10lbs constantly to move objects of up to 150 lbs. Sudden emergency situations have the potential for exposure to lifting or moving of up to 150 lbs. Frequent bending, walking, sitting squatting, reaching, and standing are required. Keyboard/computer use and/or repetitive motions may be required.
