COMMUNITY HEALTH WORKER II in Chicago, Illinois at Sinai Chicago

GENERAL SUMMARY/BASIC PURPOSE OF JOB:
Under the supervision of the Supervisor of Program Initiatives, the Community Health Worker II (CHW-II) coordinates educational opportunities and provides health education and care management services to community residents served by the Sinai Health System.

MINIMUM EDUCATION:

• High school degree or GED

MINIMUM WORK EXPERIENCE:

• 1 year of health education experience required.

KNOWLEDGE & SKILLS:

• Driver’s license preferred.
• Working understanding of Microsoft Word, Access, Excel, and PowerPoint preferred.
• Hours are flexible but will include some evening and weekend hours scheduled in advance.
• Ability to coordinate health education, under supervision.
• Ability to provide health education to large groups (20+).
• Ability to provide case management. Will work with Supervisor on complex cases.
• May assist in supervising, training and mentoring of CHW-1’s.
• Some positions may require Spanish.
