Accountabilities:

The Clinical Forms Completion Specialist is responsible for managing disability and leave documentation workflows while ensuring accuracy, compliance, and a high level of service for patients and healthcare partners. Key responsibilities include:

  • Manage and complete assigned disability paperwork cases by collecting and reviewing relevant medical information.
  • Communicate with healthcare providers and medical professionals to obtain required documentation for disability and leave requests.
  • Evaluate medical information to ensure eligibility details are accurately reflected in completed forms.
  • Enter and maintain request information within internal systems to support documentation and processing workflows.
  • Coordinate return-to-work transitions, including tracking restrictions and required workplace accommodations.
  • Respond to patient and representative inquiries through email and phone regarding Disability and FMLA paperwork.
  • Complete FMLA and disability forms using established guidelines and specialty-specific documentation requirements.
  • Ensure compliance with HIPAA regulations, privacy standards, and confidentiality requirements when handling protected health information.
  • Verify patient information using appropriate identifiers to maintain data accuracy and security.
  • Provide professional, patient-focused support throughout the documentation process.
  • Ensure completed forms are processed within established turnaround times.
  • Independently manage assigned workloads while maintaining quality, accountability, and attention to detail.
Requirements:

The ideal candidate has healthcare administrative experience, strong communication abilities, and the ability to manage sensitive medical information with accuracy and discretion. Required qualifications and skills include:

  • Previous medical office experience is required.
  • Experience as a Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), Medical Assistant, or similar healthcare certification is preferred.
  • Experience working with electronic medical records (EMR) systems is preferred.
  • Strong written and verbal communication skills with the ability to interact professionally with patients and healthcare providers.
  • Excellent attention to detail and the ability to review complex documentation accurately.
  • Customer-focused mindset with a professional and empathetic approach.
  • Strong organizational skills and the ability to manage multiple priorities independently.
  • Ability to work effectively in a remote environment with strong self-management skills.
  • Proficiency with Windows-based systems and common workplace technology tools.
  • Ability to complete tasks efficiently while maintaining accuracy and compliance.
  • Understanding of HIPAA requirements and commitment to protecting confidential healthcare information.
