We’re Hiring: Patient Care Advocate
Community Medical Group is looking for a compassionate, organized, and detail-oriented Patient Care Advocate to support our Medicare patients with appointments, referrals, transportation, and other healthcare needs.
Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m.
Location: 701 NW 27th Ave, Miami, FL 33125

What You’ll Do:
• Contact patients to schedule, reschedule, and confirm medical appointments
• Monitor referrals to ensure specialist visits are completed and clinical notes are received
• Coordinate follow-up appointments with primary care providers and in-house specialists
• Schedule and coordinate patient transportation
• Participate in daily clinical huddles to review upcoming Medicare patient appointments
• Follow up with hospitalized patients and assist with their ongoing care needs
• Support medical records and referral management
• Review patient telephone encounters, action items, and confirmed referrals
• Verify and update insurance information, addresses, and telephone numbers
• Assist patients with dental appointments and monthly over-the-counter benefit orders
• Work with HEDIS reports and other patient-care initiatives
• Answer patient calls, take messages, and provide instructions from the clinical team
What We’re Looking For:
• Associate degree in Health Sciences or a related field
• At least two years of related experience in a clinic or hospital setting
• Knowledge of healthcare and managed-care processes
• Knowledge of medical terminology and electronic health record systems
• Proficiency with Microsoft Office programs
• Strong organizational, communication, and interpersonal skills
• Ability to manage competing priorities and complete assignments independently
• Excellent judgment, initiative, resourcefulness, and problem-solving skills
• A patient-focused professional who actively looks for ways to help others
• Bilingual English and Spanish or English and Creole preferred
