The Nurse Case Manager role integrates case management, patient education, symptom management, and telephonic triage to ensure timely, safe, and effective care delivery. Collaborates with physicians, mid-level providers and all members of the multidisciplinary team within CSNF and local hospitals to ensure that patients and their families receive services which reflect our commitment to caring for the body, mind and spirit. Essential duties and responsibilities include the following:
