About the Role
We are seeking a compassionate, tech-savvy, and highly organized Retention Specialist to serve as a frontline ambassador of our home health care company. This newly created role is designed to strengthen patient satisfaction, support family caregivers, and ensure continuity of care through monthly in-home visits and hands-on assistance.
The ideal candidate will be a natural conversationalist—someone who can build trust, listen attentively, and respond with kindness and professionalism. This role is perfect for someone who thrives in the field, enjoys connecting with people, and is passionate about improving the home care experience.
Key Responsibilities

• Visit each active patient at least once per month to assess satisfaction with care and services
• Engage in meaningful conversation and identify any concerns or unmet needs
• Assist family caregivers with completing required annual education modules• Provide basic tech support during in-home visits (e.g., login issues, navigating training platforms)
• Maintain a daily schedule of patient visits across the service area
• Document visit outcomes, concerns, and follow-up needs in a timely and organized manner
• Collaborate with clinical and administrative teams to ensure continuity and responsiveness

Qualifications

• Exceptional interpersonal skills with a warm, approachable demeanor
• Ability to connect with diverse patients and caregivers with empathy and respect
• Strong organizational skills and comfort working independently in the field
• Basic tech proficiency (e.g., tablets, online portals, troubleshooting common issues)
• Experience in home health, social work, or customer service preferred• Valid driver’s license and reliable transportation required
