We are currently seeking a Registered Nurse (RN) / Licensed Practical Nurse (LPN) to join our team and assist with the health and well-being of our members.

Position Overview:
As a Registered Nurse/Licensed Practical Nurse at Sarah Care Adult Foster Care, you will play a key role in providing professional nursing care to members in compliance with Massachusetts Adult Foster Care regulations. This is a per diem position, offering flexible hours to suit your schedule. The RN will assess, plan, implement, and evaluate nursing care for individuals with chronic conditions and disabilities, ensuring their safety, comfort, and health. A critical part of this role is training and supporting caregivers to ensure they have the necessary knowledge and skills to provide the best care to our members.

Responsibilities:

• Conduct initial and ongoing assessments of members’ health status and medical needs.
• Develop and implement care plans in collaboration with physicians, caregivers, and other healthcare professionals.
• Monitor member well-being and respond promptly to any changes in members' health.
• Provide education, training, and ongoing support to caregivers on proper healthcare practices, and member-specific care needs.
• Ensure compliance with all Massachusetts regulations for Adult Foster Care (AFC) services, including licensing and documentation requirements.
• Maintain accurate medical records, including assessments, progress notes, and care plans.
• Collaborate with the clinical team to ensure high standards of care and service delivery.
• Perform other duties as assigned by the Director of Nursing or agency management.

Qualifications:

• Current Massachusetts Registered Nurse (RN) license in good standing.
• Minimum of 1 year of nursing experience, preferably in long-term care, adult foster care, or similar healthcare settings.
• Knowledge of Massachusetts Adult Foster Care regulations and healthcare laws.
• Strong clinical assessment skills and the ability to manage complex healthcare needs.
• Ability to train and educate caregivers on proper care techniques, and health monitoring.
• Excellent communication skills and the ability to work effectively with members, caregivers, and a multidisciplinary team.
• Compassionate, patient, and detail-oriented, with a commitment to quality care.
• Ability to work independently and manage a flexible schedule.
