JOB SUMMARY:
Provides basic nursing care in terms of individualized patient needs, and works to promote health, prevent disease and help patients cope with illness. Care is provided through recognized nursing techniques, procedures and established standards based on the scope of practical nursing, under the direction/delegation of the Physician. Basic nursing care includes performing routine office procedures and testing, assisting physicians as needed, participating in patient education, and communicating effectively with others.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
- Implements current nursing practice while performing direct and indirect patient care procedures by adhering to RN Scope of Practice. Administers prescribed medications and treatments in accordance with nursing standards. Supports and practices the policies and procedures as established by SouthCoast Health.
- Observes, records and reports patient’s condition and reaction to drugs and treatments to physicians. Dispenses medication as directed. Thoroughly educates patient/family about diagnostic procedures, medications, nutrition, and maintenance of health and wellness.
- Conducts pre-visit patient assessments and documents patient care observations and activities according to professional standards and procedures.
- Triages walk-in patients and telephone calls to determine emergent, urgent or non-urgent status.
- Develop patient plans of care and document care coordination and interventions while integrating quality measurements and other accrediting body requirements in order to demonstrate results and show quality improvement in patients.
- Generate report/list and proactively remind patients/families of services needed (flu shot, wellness screenings, labs, etc.)
- Maintains confidentiality in matters regarding the clinic, its patients and employees.
- Accurately and completely documents all patient observations, treatments and procedures administered, patient/family responses, medication dispensed or prescribed, hospital data in the medical record in accordance with documentation standards, policies and procedures.
- Upon initial and annual successful completion of the medication proficiency test, the nurse may administer medications according to Medication Administration policy.
- Administers immunizations and injections after reviewing immunization records, potential allergic reactions, identifies and reviews side effects and treatments.
- Processes patient referrals with required follow-through. Maintains and keeps updated referral report.
- Schedules lab work, specialist visits, completes necessary paperwork to ensure proper billing, completes physician orders. Verify and update patient data. Assist patients with completion of paperwork, as required.
- Will manage boxes in EHR of outstanding tasks, alerts, or pending actions related to patient care timely and accurately.
- Participates in code team training and immediately initiates an appropriate emergency response/procedure when codes are called (to include performing CPR, managing airways, administering medications, and coordinating evacuation procedures in case of a fire).
- Engage patients and their family members in understanding self-management plans in a culturally and linguistically appropriate manner.
- Based on the staffing needs of the department, employee will assist in other provider areas as assigned by Supervisor.
- Promotes wellness by providing patient education materials, communicating physicians’ advice and instructions on how to self-manage care.
- Coordinates patient care by triaging patient phone calls regarding urgency of appointments, and scheduling hospital admissions. Processes all patient referrals and required follow through. Maintains and keeps updated referral report.
- Participates in scheduled training programs including HIPAA compliance, online learning modules, and classroom training.
- Comes to work on a regular and consistent basis.
EDUCATION:
A graduate of an accredited school of Registered Nursing. BSN preferred.
EXPERIENCE:
Minimum of one-year clinical experience, preferably in a hospital or office practice setting.
REQUIREMENTS:
- Current Georgia RN licensure. The employee must pass initial clinical skills competencies and initial and annual medication proficiency exam (with an 80% or better).
- BLS/CPR certification. ACLS & BLS required for Cardiology Department RN’s.
KNOWLEDGE:
- Knowledge of professional nursing theory, regulations and practices to give and evaluate patient care.
- Knowledge of policies and procedures related to infection control, environmental safety and patient confidentiality.
- Knowledge of medical charge reporting/records.
SKILLS:
- Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care.
- Skill in appropriate use of universal precautions, safe workplace and confidentiality methods.
- Skill in identifying problems and recommending solutions.
- Skill in preparing/maintaining records, writing reports and responding to correspondence.
- Skill to use of computer.

ENVIRONMENT & CONDITIONS:
- Primarily clinical setting.
- May be subject to infectious materials through the collection or handling of body fluids (i.e., blood, urine, sputum), caring for patients with highly infectious/contagious diseases.
- Initial Hepatitis B screening and initial and annual TB screening/testing is required.
- Occasional stress due to workload pressure.
PHYSICAL/MENTAL DEMANDS:
- Requires standing and walking for long periods of time.
- Occasionally lifts and carries items weighing up to 20 pounds.
- Frequently required to talk, hear, eye/hand coordination as well as keyboarding.
- Occasionally stooping, lifting, grasping, pushing/pulling and grip strength.

This position shall have full access to information in a patient’s medical record, in order to provide quality patient care. Patient information is to be held strictly confidential. Patient information will not be used or disclosed for any purpose other than the performance of job responsibilities and duties.
